Position/Title: Consultant, Triple Elimination (TE) Program – Indonesia

Program Background

The World Health Organization (WHO) has set a global goal to eliminate mother-to-child transmission (MTCT) of HIV, syphilis, and hepatitis B (HBV) by 2030 through integrated antenatal care (ANC) screening, treatment, and infant prophylaxis – an approach known as Triple Elimination (TE). Indonesia has made substantial progress toward this goal, including well-established national policy frameworks, expanded geographic coverage of screening, and rising HBV birth dose immunization coverage. However, HBV screening continues to lag behind HIV and syphilis, subnational coordination across Ministry of Health (MoH) departments remains fragmented, TE data systems remain siloed across disease programs, and access to integrated diagnostic approaches – including triple combination tests – and to Tenofovir prophylaxis for HBsAg-positive pregnant women remains limited.

To help close these gaps, CHAI proposes to support the Government of Indonesia in developing a scalable, government-led model for TE integration at the subnational level. The initiative will be implemented in Bogor Regency and Bekasi City, West Java and is organized around four workstreams: (1) strengthening coordination and alignment across MoH departments; (2) supporting the implementation research and pilot of triple combination tests; (3) improving integration of TE data systems, including with SatuSehat; and (4) strengthening the health workforce through a Center of Excellence clinical mentoring model. The initiative builds on CHAI’s existing MNCH platform in West Java and is intended to generate evidence and tools applicable to other provinces and countries in Southeast Asia.

Purpose of this Consultancy

This consultancy covers the inception phase of the initiative: the situational analysis and selection of pilot facilities, the co-design workshop with government and partners, the detailed implementation plan for 2027 and 2028, and preparation for a proposed pilot of triple combination testing. The design and scope of the pilot remain subject to confirmation with partners and the Ministry of Health, and the preparatory work will be taken as far as those decisions allow.

The role combines in-country coordination with technical execution. The consultant will lead engagement with Ministry of Health units and district health offices, run the situational analysis, co-lead the design workshop and the implementation plan that follows from it, and take forward the technical and regulatory preparation for the proposed pilot. The consultant will work closely with CHAI’s global and regional triple elimination teams, which provide technical review and input across the program.

CHAI is therefore looking for a candidate able to operate at program management level, working with limited supervision and representing CHAI to government counterparts and partners.

CHAI maintains ultimate responsibility for the program, including final decisions on program approach, in coordination with the program donor.

Scope of Work and Deliverables

The work is organized in three areas, aligned to the program workplan. CHAI drives delivery across all three, working with program partners on jointly owned activities, and the consultant leads this work on CHAI’s behalf. All deliverables are subject to review and acceptance by CHAI. The balance of work across the areas will depend on the start date and on progress made before the consultant joins.

1. Situational analysis, co-design, and implementation planning

• Assessment of triple elimination service delivery across an agreed set of facilities in Bogor Regency and Bekasi City, including the assessment approach and facility assessment tools, and drawing on existing CHAI program data. This should cover guideline availability, training availability, the care algorithm and actual implementation flow, financing, and recording and reporting flows.
• Assessment report and presentation summarizing key findings and recommendations, and a shortlist of priority primary health centers selected against agreed criteria, with the selection rationale documented.
• Support co-design workshop planning and materials, and co-facilitation of the workshop alongside partners.
• Detailed two-year implementation plan for 2027 and 2028, built from the co-design outputs and agreed partner roles, including activities, timeline, milestones, and an updated budget.

2. Preparation for the proposed triple combination test pilot

• Documented scoping of triple combination test feasibility and requirements in Indonesia, continuing work already under way, and contribution to partner alignment on protocol objectives and algorithm design.
• Supplier engagement on donation terms, product specifications, and regulatory status for the candidate tests, with CHAI’s global team.
• Study protocol for the proposed pilot, adapted from CHAI’s existing materials, and the accompanying submission to the relevant institutional review board.
• Complete Special Access Scheme documentation package for import of the tests, compiled and version-controlled, including Ministry of Health endorsement, submitted for review.

3. Coordination and reporting

• Stakeholder engagement and coordination plan covering the Ministry of Health pharmacy, maternal and child health, HIV, and hepatitis units, and West Java Provincial Health Office, and the district health offices in Bogor Regency and Bekasi City, with summary reports from coordination meetings held at national and district level.
• Monthly progress reports against the workplan, setting out status, key risks, and mitigation actions, and providing inputs to CHAI’s reporting to the funding partner.

Budget and Disbursement

The budget for the work amounts to IDR 170.000.000 (inclusive of tax) for six months, covering professional fees and all applicable income tax. This amount excludes expenses related to necessary activities such as travel, meetings, and workshops (including stakeholder coordination and training events), which will be covered separately. Payment will be disbursed by deliverable, upon acceptance of each deliverable by the CHAI Indonesia Senior Program Manager. The national meeting budget will be managed separately by CHAI Indonesia.

Timeline

The consultancy is expected to run over a 6-month period subject to adjustment based on the outcomes of collaborative design and alignment with the government. It is renewable subject to performance and confirmed funding. Timelines within the consultancy may be adjusted in line with government processes and the outcomes of co-design.

Qualifications

• Bachelor’s degree in public health, medicine, health sciences, or a related field; master’s degree preferred.
• Minimum of 5 years of relevant professional experience in HIV, viral hepatitis, MNCH/PMTCT, or related public health programs in Indonesia, including at least two years in a program management or coordination role.
• Experience designing and conducting operational research within Indonesia health, including protocol development and submission for ethical clearance.
• Experience preparing regulatory or import documentation for health products in Indonesia, including the Special Access Scheme, or demonstrated ability to navigate these processes.
• Experience coordinating across multiple government counterparts, including Ministry of Health directorates and provincial and district health offices, and within multi-partner initiatives.
• Demonstrated experience managing workplans, budgets, and partner relationships on donor-funded programs.
• Familiarity with Indonesia’s Triple Elimination policy framework (MoH Regulation No. 52/2017), the Integrated Primary Health Care (ILP) reform, and ANC service delivery in the public and private sectors.
• Familiarity with health information systems such as SatuSehat, Simpus, or district-level dashboards.
• Strong stakeholder engagement skills across technical, government, and community stakeholders.
• Excellent written and verbal communication skills in both Bahasa Indonesia and English.
• Ability to work independently, manage multiple priorities across workstreams, and travel to project sites in Bogor Regency and Bekasi City, West Java, as needed.

Advantages:

• Experience with the introduction or evaluation of rapid diagnostic tests in routine service delivery.
