REQUEST FOR QUOTATION TRAVEL AGENT SERVICES
BACKGROUND
ThinkWell as the implementing agency of Health Financing Activity (HFA) and Strategic Purchasing for Primary Health Care (SP4PHC) seeks Travel Agent Services to support our activities. The Health Financing Activity (HFA) is a five-year contract funded by USAID/Indonesia to provide technical assistance to the Ministry of Health for strengthen local capacity in financial analyses, stakeholder engagement, learning, and decision-making. Strategic Purchasing for Primary Health Care (SP4PHC) is a nine-year project funded by the Bill and Melinda Gates Foundation that is supporting countries to improve how funds are allocated to healthcare providers to achieve the goal of universal health coverage. The project is working to increase access to high-quality primary health care services, particularly for poor and vulnerable populations, by supporting national and subnational governments to establish how best to engage and pay health care providers.

- Lodging/Hotel Accommodation (domestic and international)
- Transportation/Rental Vehicle
- Travel document arrangement (Visa/Passport/Travel Insurance)
- After Hours Emergency Service Assistance 24 hours
- Travel Management Report
- Airport Assistance
- Corporate Implant Travel
- Account Management
- Meeting, Incentive, Convention and Events (MICE)
All quotations are requested with the following information:
- Company information, including company profile, vendor’s physical address, full legal name, email address, website and contact person.
- Company organogram and staff details (if any)
- Company registration certificate that clearly demonstrates legal registration in the category being applied for.
- Valid tax registration certificate
